Therapy services are provided via telehealth only and are limited to residents physically located in the states of Minnesota, Texas, or Oklahoma at the time of service.

Purpose of the Website

This website is intended to provide general information about the services offered by By Design Behavioral Health, PLLC, including individual teletherapy for adults (18+), appointment scheduling, and access to telehealth services. The information provided is not a substitute for professional diagnosis, treatment, or emergency intervention.

Eligibility and Scope of Services

The Practice provides individual outpatient therapy for adults only. We do not provide emergency services, crisis intervention, or services for individuals under the age of 18.
If you are experiencing a crisis or mental health emergency, please contact 911 or go to the nearest emergency room. You can also contact the Suicide & Crisis Lifeline at 988.

Telehealth Services

All therapy sessions are conducted via a secure, HIPAA-compliant video platform. By engaging in telehealth services, you acknowledge the following:

  • You must be 18 years of age or older and physically present in Minnesota, Texas, or Oklahoma at the time of each session.
  • You are responsible for ensuring a private, secure, and distraction-free environment during sessions.
  • You consent to receive mental health services using telecommunication technologies.
  • You understand the risks and limitations of telehealth, including potential technical failures or breaches despite best efforts to maintain security.

No Provider-Client Relationship via Website

Use of this website does not establish a therapist-client relationship. Such a relationship is only formed after a thorough intake process, mutual agreement, and completion of all necessary forms, including informed consent for telehealth treatment.

Privacy and Confidentiality

The Practice is committed to protecting your privacy in accordance with HIPAA and applicable state laws. Please review our [Privacy Policy] for details about how we collect, use, and protect your personal information.

Scheduling and Cancellations

Appointments may be scheduled through a secure client portal or by contacting the Practice directly. A minimum of 24 hours’ notice is required for cancellations. Missed or late-canceled appointments may be subject to the late cancelation or no-show fee.

Fees and Payment

Fees for services will be disclosed prior to the start of treatment. Payment is due at the time of service unless otherwise agreed upon. The Practice only accepts self-pay.

INDEMNIFICATION

c. your access or use of the Website;
d. any action taken by us either as part of our investigation of any suspected breach of the Term of Use or as a result of our finding or decision that a breach of this Term of Use has occurred; and

Attorneys’ Fees
